a beat in the last 10 min, newest per host+kind, not stopped) and
retiredProcesses[]; 45s stale and level only consider current ones
- heartbeat writer marks stopped_at on graceful exit (migration 0043),
upsert clears it; pure api role now stops its writer on SIGTERM
- scheduled job system.heartbeat_cleanup (maintain, 1h) deletes rows
without a beat for 24h and samples older than 7 days
- status page: per-role alarms ("io worker 45 秒没有心跳"), collapsible
retired instances; glossary/runbook updated
- bump all members to 0.2.2, CHANGELOG [0.2.2]

OutboxRelay gains a `mirror` option: listed types are claimed even without a
task route and, after the claim transaction commits, published in order to
the realtime bus (publish failures are logged, delivery state unaffected).
The worker mirrors feed.*, decision.*, download.*, organization.* and
subscription.* (feed.due excluded) with their business-id payloads, so the
home page, inbox, task stream and feed tables finally receive them. Feed
tables refresh on feed.fetched / feed.health_changed; the task stream also
listens to feed and decision events.

- downloads.ts observe: re-read download_task FOR UPDATE inside the tx and merge
from the locked row; qB sync could overwrite imported back to importing when
verifyAndImport committed between the sync read and write (E8 flake).
- agent finalize: accept summaryPrefix so the recovery verdict is written with
the terminal status in one update (E6 read a budget_exceeded run before the
"未恢复:…" annotation landed); recover-summary step kept, now idempotent no-op.
- organize.ts: drop local slotComplete and the once-subscription completion
write; completion/history are decided only by applySlotEvent(verify_passed)
(W5-C follow-up, removes double write). Manual slot organize uses the shared
subscriptions.slotComplete.
- vitest: server split into unit (parallel) and pg (file-serial, groupOrder 1)
projects; packages/tasks file-serial; explicit worker concurrency in e2e.
- docs: W6 stabilization record in 04-v0.1-acceptance.md.
Test expectations unchanged. Full `deno task ci` x3 green: 369s / 341s / 337s.

- POST /organization-records: sourcePaths must resolve (realpath) under a
registered library_root or downloader path-mapping host path; `..` and
symlink escapes rejected (400), dryRun included. Move requires an admin
session, or an API key with an automation_grant covering the entity or a
matching one-time approval (consumed on use), else 403. Manual Move from a
managed download_task with unsatisfied H&R returns 409 and opens a
decision; never silently downgraded to Copy.
- Series slot completion: total episode count is unknown while units are
provisional, so the slot is not satisfied and the subscription keeps
tracking; coverage is registered per actual file (creating provisional
episode units), and already_in_library files count as verified presence.
- POST /engine/runs/{id}/retry (admin): re-enqueue by original task name and
raw input with idempotency key retry:<id>:<n>, audited; retry button on
the run detail page.
- E7 regular series e2e, E8 AT24 tag removal / task disappearance e2e.
- All workspace members and root set to version 0.1.0; CHANGELOG.md.

tasks_run_meta/tasks_fact/tasks_op_ledger move into packages/db (if not exists, old
tasks_migration tables dropped); packages/tasks/migrations and migrateTasksTables
removed, tests migrate their schema with @umt/db. agent_run_event(run_id, seq, type,
at, payload) with (run_id, seq) primary key; types regenerated, db-sync covers it.

The relay only looked at published_at and ignored the ADR-002 §2.1 columns
added in db 0002_foundation: rows stayed status='pending' forever after
dispatch, failures were retried every tick without backoff, and the
row-level idempotency_key was never used as the engine enqueue key.
- claim status='pending' AND next_attempt_at<=now()
- enqueue with idempotency_key (fallback outbox:<id>)
- success: status=dispatched, published_at, attempts+1
- failure: attempts+1, exponential backoff (cap 5 min), last_error; dead at maxAttempts
- test helper table now mirrors db 0001+0002; tests cover backoff and dead
